When offering telehealth services, it’s important to inform clients about the benefits of telehealth sessions, potential risks and their rights to privacy and confidentiality.
This simple 2-page consent form template lists, in easy-to-understand words, the generic aspects of telehealth consultations that clients need to know. It is advised that you edit it to suit your needs and provide some more specific information (e.g., the video conferencing platform you use).
