My Health Online is an Australian dietitian-led clinic offering telehealth care for adults living with chronic conditions. The service combines bulk-billed telehealth appointments with a structured Lifestyle Program, and serves two distinct audiences: patients seeking accessible, ongoing support, and GPs referring patients under a Chronic Disease Management Plan. The design reflects that clinical credibility while remaining approachable and clear, with a straightforward structure that helps each visitor quickly find the pathway relevant to them. Separate patient and referrer journeys are built into the site from the ground up.
